Im Iran ist Schwulsein per Gesetz verboten, Drogenabhängige und Prostituierte gibt es offiziell nicht. Da diese Risikogruppen angeblich nicht vorhanden sind, fehlt in der islamischen Republik auch die Aufklärung über Aids. Immerhin werden die Infizierten nicht völlig im Stich gelassen ...

Forscher haben gleich 17 neue Antikörper entdeckt, die sehr effektiv gegen Aids-Viren wirken. Sie könnten die Suche nach einem Impfstoff gegen HIV vorantreiben. Rare strain of AIDS virus moves beyond Cameroon

A very rare strain of AIDS virus previously found only among a few people in Cameroon has most probably spread outside the West African country, according to a case reported by The Lancet on Friday.

The first identified infection with the so-called "group N" strain of the human immunodeficiency virus (HIV) was found in 1998 in a Cameroonian woman who had progressed to AIDS.

Since then, more than 12,000 HIV-infected patients living in Cameroon have been tested for group-N infection, but only 12 cases, including two couples, have ever been found. mehr...

HIV - Erste klinische Studien am Menschen beginnen demnächst

Forscher haben erstmals mehrere Kombinationsimpfstoffe gegen Aids getestet, die auch vor einer aggressiven, resistenten Virenmischung schützen. In Versuchen an Rhesusaffen reduzierten die gentechnisch hergestellten Vakzinen das Infektionsrisiko der Tiere bei einmaligem Kontakt mit Affenaids-Viren (Simian Immunodeficiency Virus, SIV) um 80 Prozent. Man habe mindestens vier Verabreichungen von Virenlösungen benötigt, um die Hälfte der geimpften Affen mit der HIV-ähnlichen Krankheit zu infizieren. Die trotz Impfung infizierten Tiere hätten zudem deutlich weniger Viren im Blut gehabt als die nicht geimpften, berichten die Forscher im Fachmagazin "Nature" ...

This will be a busy year for Rwanda's health centres as the country attempts to reach its goal of medically circumcising 50 percent of men by June 2013 as part of HIV prevention efforts.

"We plan to extend free male circumcision services to all men in Rwanda - we are targeting two million circumcisions by 2013," said Simoni Kanyaruhango, head of the national male circumcision programme at the Rwanda Bio-Medical Centre. "The programme has, under the sponsorship of the Global Fund [to fight AIDS, Tuberculosis and Malaria], extended the necessary kits ... to all district hospitals, which will in turn offer the service free of charge to the public."

The free male circumcision programme began in October 2011, and officials at the Ministry of Health say demand is growing.

"Here we carry out circumcisions every weekend but we are looking at including the working days as the demand is increasing by the day," said Christian Ntizimira, director of Kibagabaga Hospital in the capital, Kigali.

A large randomized controlled trial in Kenya, South Africa and Uganda found that medical male circumcision can reduce a man's risk of contracting HIV through vaginal intercourse by almost 60 percent.

In order to reach 80 percent coverage - a target set by UNAIDS and the World Health Organization (WHO) under a new plan [ http://www.plusnews.org/report.aspx?reportid=94404 ] to accelerate medical male circumcision in eastern and southern Africa - Rwanda would need to circumcise 1,746,052 men; at present, some 15 percent are circumcised.

However, with a severe shortage of highly trained medical staff - according to WHO, [ http://www.who.int/whosis/whostat/EN_WHS2011_Full.pdf ] Rwanda has just two doctors per 100,000 population - the goal is unlikely to be met unless lower cadre health workers are involved in the campaign.

Simpler techniques

At present, the programme is using circumcision surgery, the only WHO-approved method.

The government is hoping for WHO approval of a device known as the "PrePex system", which delivers "bloodless" male circumcision [ http://www.plusnews.org/report.aspx?reportid=91919 ] and would reduce the need for a sterile environment, anaesthetic and highly trained medical personnel. The PrePex system works through a special elastic mechanism that fits closely around an inner ring, trapping the foreskin, which dries up and is removed after a week.

"This device has been clinically studied and found effective. We are only awaiting approval from the World Health Organization Technical Advisory Group on technical innovations in male circumcision," said Vincent Mutabazi, lead investigator in the PrePex Clinical study.

"With WHO approval of the device, we could perform male circumcisions anywhere, any time or even run mobile clinics out to remote communities rather than have men travel long distances for the circumcisions," said Agnes Binagwaho, the Rwandan Minister of Health.

Education gaps

Messages on male circumcision have been widely broadcast using print and electronic media, and health centres are also being used to promote the programme.

However, many in the target population remain unaware or afraid of the procedure. "I know about it of course and I appreciate its importance, but what would happen if I don't heal properly or even heal at all?" asked James Nkuusi, a restaurant owner in Remera, a Kigali suburb. "Besides, my wives are used to me the way I am now - my size, you know. If I got circumcised it would be difficult for me to satisfy them I guess, and I would never let that happen."

Rwanda Bio-Medical Centre's Kanyaruhango said the government had made significant progress in demystifying the procedure. It is also being careful to emphasize that male circumcision must work in conjunction with other HIV prevention methods to be successful.

"Male circumcision should only be one element of a comprehensive HIV prevention package, which should include the promotion of condom use, the provision of HIV counselling and testing services and treatment of sexually transmitted infections. And this is what we emphasize," said Kanyaruhango.

Ein interessanter Artikel in der Zeitung '20 Minuten' von heute:

"In einem neuen Buch, das sich mit der Verbreitung von Aids beschäftigt, stellen die Wissenschaftler Craig Timberg und Daniel Halperin einen starken Zusammenhang zwischen der weltweiten Ausweitung der Krankheit und dem Kolonialismus her. Ein Auszug aus «How the West Sparked the Aids Epidemic and How the World Can Finally Overcome It» wurde kürzlich in der «Washington Post» veröffentlicht. Darin beschreiben die Autoren, dass es immer mehrere gemeinsam auftretende Faktoren braucht, damit aus einer Infektion eine globale Pandemie wird. Es brauche ein ganz bestimmtes Zeitfenster – dieses sei im Kolonialismus zu finden."

"Ausgebrochen ist die Epidemie in den Hinterländern im Südosten Kameruns. Laut wissenschaftlichen Ergebnissen muss dies zwischen 1880 und 1920 passiert sein. Ursprünglich waren Schimpansen mit dem Virus infiziert. Auf den Menschen ging dieser über, als ein Jäger einen Schimpansen tötete, mit dem Blut des Affen in Berührung kam und sich dabei mit dem HI-Virus angesteckt hatte."

Es war also eine einzelne Aktion. Die passierte innerhalb eines Zeitraums von 40 Jahren.


"In einem fieberhaften Rennen um Wohlstand und Ruhm hatten sich europäische Mächte auf den afrikanischen Kontinent gestürzt. Hierfür waren sie auch in Gegenden eingedrungen, die bislang kaum von Menschen bereist worden waren."

Kaum Menschen? Und die vielen Millionen von Einheimischen?


"Die erste Blutprobe, in der der HI-Virus festgestellt werden kann, stammt aus dem Jahr 1959 aus der kongolesischen Stadt Kinshasa. Aufgrund der Kolonialisierung des Kontinents sei diese Reise vom südöstlichen Kamerun nach Kinshasa überhaupt möglich gewesen. Durch Handelsrouten konnte der HI-Virus den Ngoko-Fluss entlang bis nach Kinshasa gelangen."

Und ich dachte immer, dass selbst bevor die Weissen Strassen und Eisenbahnen bauten, die Einheimischen ueber ganz Afrika einen regen Handelsverkehr hatten.

Da lag ich dann wohl falsch :!:

Ein blose Berührung mit Blut verursacht noch keine Ansteckung. Da muß eine andere Ursache gewesen sein.

Handel wurde auch schon vor dem "Erscheinen" des weißen Mannes getrieben. Wenn auch nicht zu bestreiten ist, dass die Verbreitung von Krankheiten durch Einführung von Technologie (Autos, Eisenbahn, Schifffahrt) wesentlich schneller von statten geht.

Interessant ist der Beitrag dennoch, besonders unter dem Aspekt, wie und vor allem wie schnell sich eine Seuche ausbreiten kann.

Das aus Afrika stammende HI-Virus ist im Laufe seiner Evolution mindestens vier Mal auf den Menschen übergesprungen: Verschiedene in Rhesusaffen, Schimpansen, Gorillas oder Mangaben zirkulierende Affenvarianten entwickelten sich nach ihrem Sprung über die Artgrenze zu den verschiedenen Untergruppen des menschlichen Immunschwächevirus. Mediziner interessieren sich vor allem für die Veränderungen zwischen der für Menschen fast immer harmlosen Affenform SIV (Simian Immunodeficiency Virus) und den aus ihnen abgeleiteten Untergruppen der heute virulenten HIV. Nun berichtet ein internationales Forscherteam, dass der Unterschied zwischen Affenviren und seinem wichtigsten menschlichen Ableger so subtil wie weit reichend ist ...

An epidemic of a deadly strain of _Salmonella_ has swept across the whole of Africa by "taking advantage" of the spread of HIV, according to an international team of researchers. Their study, published in Nature Genetics, is the 1st to identify the separate cases as a single epidemic. 1 in 4 people in Africa infected with the strain died.

It is thought to be the 1st time a single strain of an infection has spread so widely in the wake of HIV. Cases of this form of invasive non-typhoidal salmonellosis have been recognized in Africa for more than a decade. It causes fever, headaches, respiratory problems, and sometimes death.

The research team analyzed the genetic code of 179 strains of the serotype from different parts of Africa and the rest of the globe. Using techniques similar to a large-scale DNA paternity test, they were able to construct the strain's "family tree" and then how it spread. It happened in 2 waves. The 1st started in south eastern Africa about 52 years ago and the 2nd wave started 35 years ago from the Congo Basin.

Prof Gordon Dougan, from the Sanger Institute in Cambridge in the UK, told the BBC: "It quite clearly parallels the emergence of HIV in Africa." HIV attacks the immune system and leaves people more vulnerable to other infections. It is thought the strain of _Salmonella [enterica_ serotype] Typhimurium took advantage of this weakness and spread. The research team said the bacterium was given the chance to "enter, adapt, circulate and thrive."

There is poor monitoring data for the disease across the whole of the continent, but Prof Dougan said it was affecting "thousands and thousands" of people and that 98 percent of adult cases were in people with HIV.

Prof Brendan Wren, London School of Hygiene and Tropical Medicine, said this spread of _S._ Typhimurium had been different to that of other infections commonly associated with HIV, as it had been a single epidemic "people were completely unaware" of and there "were not really any other examples" of that happening.

Dr Melita Gordon, a gastroenterologist at the University of Liverpool, said: "It's the 1st time this has been described right across a continent in such an obvious way." She added, "The highest mortality associated with the disease is 80 percent. What's happened over the years is mortality has fallen down and down and down to between 20 percent and 25 percent as doctors inside Africa recognise it."

The genetic analysis also showed the strain was resistant to the 1st choice antibiotic, chloramphenicol, which means more expensive drugs would be needed to treat the infection.

It is thought that improving HIV treatment across Africa could reduce the prevalence of the _Salmonella_ infection, as it would reduce the number of people with vulnerable immune systems. However, the researchers urged "vigilance" in case the _Salmonella_ strain mutated again to become able to infect people with healthy immune systems.

Commenting on the study, Prof Brendan Wren, from the London School of Hygiene and Tropical Medicine, told the BBC: "It's actually quite a huge problem and it seems to be getting worse because there are many susceptible people, it's got a grip in Africa. HIV, I think it's fair to say, provided a springboard for it to take off." However, he thought the disease was "near its peak" as HIV was more controlled in other continents giving it little room to spread.

"A highly invasive form of non-typhoidal _Salmonella_ (iNTS) disease has recently been documented in many countries in sub-Saharan Africa. The most common _Salmonella enterica_ serovar causing this disease is Typhimurium (_Salmonella_ Typhimurium). We applied whole-genome sequence-based phylogenetic methods to define the population structure of sub-Saharan African invasive _S._ Typhimurium isolates and compared these to global _S._ Typhimurium populations.

Notably, the vast majority of sub-Saharan invasive _S._ Typhimurium isolates fell within 2 closely related, highly clustered phylogenetic lineages that we estimate emerged independently about 52 and about 35 years ago in close temporal association with the current HIV pandemic. Clonal replacement of isolates from lineage I by those from lineage II was potentially influenced by the use of chloramphenicol for the treatment of iNTS disease.

Our analysis suggests that iNTS disease is in part an epidemic in sub-Saharan Africa caused by highly related _S._ Typhimurium lineages that may have occupied new niches associated with a compromised human population and antibiotic treatment."

One clinical observation regarding this remarkable epidemic is that, in this cohort of patients, the presentation is distinct from those both of gastroenteritis due to salmonellosis and from typhoid fever. The illness tends to be characterized by a nonspecific fever that can be indistinguishable from malaria and only in rare cases is accompanied by diarrhea. In some parts of Africa, this invasive salmonellosis has surpassed _S._ Typhi as the dominant cause of invasive disease due to salmonellosis. - Mod.LL
