A teen from South Kazakhstan oblast has been diagnosed with Crimean-Congo hemorrhagic fever (CCHF), Tenginews reports, citing the press office of the Regional Consumer Rights Department.
On 21 Jul 2014, a 14-year-old teenager came to a local hospital feeling sick after being bitten by a tick on 15 Jul 2014. The boy removed the tick himself and did not go to the hospital until he started feeling bad. He was transferred to the Regional Hospital after the tests confirmed CCHF.
At the moment, the boy's condition is considered satisfactory. It has been the 4th confirmed case of CCHF in the region since the beginning of the year [2014]. Earlier, on 18 Jul 2014 a resident of Yenbekshi District was hospitalised with similar symptoms. The man said that a couple days before, he had removed a tick from his hip. After being diagnosed with CCHF, the patient was transferred to a hospital in Shymkent. He is currently under a doctor's supervision at the hospital, and his condition is considered satisfactory.
Since the beginning of the year [2014], there have been 24 patients with suspected CCHF infection. In 2 years, there has not been a single lethal case.
"Last year [2013], there were 4435 tick bites registered. This year [2014], thanks to chemical control, there have been 3003 cases [of tick bites]," the Department reported.
According to WHO, CCHF is a common virus "primarily transmitted to people from ticks and livestock animals." Although there is no vaccination available, the antiviral drug ribavirin is used to treat bitten patients. CCHF has a mortality rate from 10-40 per cent.

Muhammad Shah, the district health officer (DHO), told The Express Tribune that 2 farmers in the Zhob district of Balochistan have died after suffering from Crimean-Congo haemorrhagic fever (CCHF), also known as 'Congo virus' [Crimean Congo hemorrhagic fever (CCHF) virus].
At least 4 people, including a woman, were infected with the virus in the small village of Killi Blump near Meena Bazaar, located 40km from Zhob city.
The deceased were identified as a 30-year-old man, who died in the Combined Military Hospital, Zhob, and another man, age 60. Meanwhile, the 30 year old was admitted to Quetta's Jilani Hospital and discharged after receiving treatment. A woman was also admitted to Fatima Jinnah TB Sanatorium, according to DHO Shah.
The village where the deaths took place consists of 12 to 13 houses. DHO Shah said the 2 farmers contracted the disease in mid-July [2014] and the virus was confirmed after blood tests [were done]. While the 1st man was sent to Quetta for treatment, the 2nd was sent to Multan.
The medical superintendent of the District Headquarters Hospital Zhob, Abdul Rehman, said animals in the village have been vaccinated, as well as family members of those affected in order to avoid the spread of the virus. The virus is primarily transmitted to people from ticks and livestock animals. Human-to-human transmission can also occur.

CCHF virus is a tick-borne virus in the genus _Nairovirus_, family _Bunyaviridae_. The virus can cause severe viral hemorrhagic fever outbreaks, with a case fatality rate of 10-40 per cent. The virus is transmitted by _Hyalomma_ spp. ticks or through contact with infected human blood or animal blood and tissues during and immediately after slaughter. The majority of cases have occurred in people involved in the livestock industry, such as agricultural workers, slaughterhouse workers, and veterinarians. Exposure in health care facilities also occurs. The length of the incubation period depends on the mode of acquisition of the virus. Following infection by a tick bite, the incubation period is usually 1-3 days, with a maximum of 9 days.

Another case of Crimean-Congo haemorrhagic fever was recently registered in Georgia's Shida Kartli region. One of the locals in the region was revealed about a week ago to have the symptoms of the disease.
Vaccination of Crimean-Congo haemorrhagic fever is being held in Shida Kartli region.
This April 2014, 2 people died from this disease in Georgia, and 13 cases of infection with Crimean-Congo haemorrhagic fever [virus] have been identified in Khashuri region of the country.
The disease poses a risk to human life, but in case of timely visit to the doctor, the patient can be saved, doctors say.
Currently, 5 people are undergoing treatment for the disease in the Infectious Diseases Hospital of Tbilisi, one of whom is a 6-year-old girl.
The National Center for Disease Control and Public Health of Georgia noted that the epidemiological situation in the country at present is not a cause for concern.

Whereas infection in birds mostly tends to be abortive, ostriches are important hosts. In the mid 1990s, South Africa saw a CCHF outbreak at an ostrich abattoir.
About 30 countries in Africa, Asia, and Europe have reported CCHF. The tick is found no further than 50 degrees north latitude, which cuts across Russia, Ukraine, central Europe, and France; the latter 2 regions are not considered to be at any immediate risk; the _Hyalomma_ spp. ticks are present, but there is no serological evidence of CCHF. The distribution of known cases across the Middle East and Africa is patchy; ticks are present in the whole of this region, but in many countries, it seems harmless. In Africa, only South Africa, and from time to time Mauritania, face ongoing outbreaks; countries such as Tanzania and Uganda have not reported any recent outbreaks; and countries such as Angola and Algeria seem not to be at risk.
Only 4 countries report more than 50 cases per year: Iran, Russia, Turkey, and Uzbekistan (although, it could easily be the case that these countries simply have particularly efficient surveillance systems backed by proper diagnostics).

The 2nd Congo virus case of the year [2014] recently surfaced in the city [Karachi, Sindh province]. Luckily, the patient is said to be in a stable condition, The Express Tribune learnt on Monday [8 Sep 2014].
The patient who is admitted in the Jinnah Postgraduate Medical Centre (JPMC) is a 17-year-old student and a resident of Shah Faisal Colony.
The family, who could not afford the cost of private medical care, had taken the boy to a charitable hospital in Korangi in a critical condition. Sources said that the boy was referred to the JPMC as the administration had told the family that there was no bed available for him. The hospital administration had suspected that the boy was suffering from the Crimean-Congo hemorrhagic fever and referred him to the JPMC on 4 Sep [2014].
The laboratory tests, which were sent to the Aga Khan University Hospital, confirmed on 6 Sep [2014] that the patient had contracted the Congo virus.
Weeks after the 1st case was reported in the city, the provincial health authorities have failed to take adequate measures to stop the virus from spreading.
The history of the latest patient shows that he used to play in a ground located near a cattle pen. The healthcare providers believe that the boy was infected during his visits to the cattle pen.
According to Dr Seemin Jamali, the joint executive director of the JPMC and the in-charge of its accidents and emergency ward, the boy was brought to the hospital in a very critical condition, adding that there was blood in his urine and vomit. "Yes, he is a positive Congo virus case," Dr Jamali confirmed to The Express Tribune. She said that the boy is being kept in the isolation ward, adding that all precautionary measures are being taken.
Despite the high mortality rate, Dr Jamali seemed confident that the boy will survive. "He is in stable condition and is gradually recovering," she added. This is the 1st case in one of the city's biggest health facilities where a patient with Congo virus is said to be in stable condition.
Earlier, a 24-year-old man, the city's 1st case of the year, died in a private hospital in the 4th week of August [2014]. Kashif was a resident of Gulberg town, Federal B Area, Union Council No. 1.
The man was a butcher by profession. The city's top health officials had convened meetings and claimed that they had issued warnings to slaughterhouses and cattle pen owners regarding the virus. They also claimed that all precautionary measures were being adopted so that more cases could be avoided.
Health officials of the Karachi Metropolitan Corporation also conducted a survey of 2000 houses around he man's house to check for possible threats of the virus being present in the area.
Health care providers fear that the situation seems very critical as 2 cases have surfaced within 15 days and thousands of animals are being brought to the city every day.

The man was a butcher by profession. The city's top health officials had convened meetings and claimed that they had issued warnings to slaughterhouses and cattle pen owners regarding the virus. They also claimed that all precautionary measures were being adopted so that more cases could be avoided.
Health officials of the Karachi Metropolitan Corporation also conducted a survey of 2000 houses around he man's house to check for possible threats of the virus being present in the area.
Health care providers fear that the situation seems very critical as 2 cases have surfaced within 15 days and thousands of animals are being brought to the city every day.

Die Volksblad reports that a sheep farmer from Namibia, age 40, died on Tuesday [16 Sep 2014] from Crimean-Congo haemorrhagic fever (CCHF).
CCHF is, like Ebola, a viral haemorrhagic fever and the symptoms are similar. In both cases the patient needs to be quarantined to prevent the spread of the virus.
The man was in Upington to attend a wedding when he started feeling ill and was taken to the emergency room at the local hospital. The doctor gave him an injection for tonsillitis and sent him home, Die Burger reported. By Friday [12 Sep 2014], during the wedding and feeling worse, he returned to the hospital where CCHF was dismissed since his blood platelet count was normal. That evening, Dirk Kok reports for OFM, the man was flown to Bloemfontein by air ambulance service Aerocare. The pilot and paramedic on board wore special medical suits to protect them from contracting the highly contagious disease. Despite the hospital staff's best efforts, the man passed away on Tuesday evening [16 Sep 2014].
According to the print edition of Die Burger (18 Sep 2014), family are now struggling to get his body back to Namibia as the corpse needs to be sealed to prevent transmission. Like with Ebola, human-to-human transmission can occur from close contact with the blood, secretions, organs, or other bodily fluids of infected persons, even after death.
People with CCHF suffer from flu-like symptoms and haemorrhaging. According to the WHO, CCHF [virus] is primarily transmitted to people from ticks and livestock animals.
The virus is transmitted mainly by _Hyalomma_ ticks, which have distinctive brown and white bands on their legs and are known in South Africa as bont-legged ticks (Afrikaans: bontpootbosluise). _Hyalomma_ ticks are widely distributed in South Africa, but tend to be most numerous in the drier, north-western parts of the country in the Karoo, western Free State, Northern Cape and North-West Province, generally in the rural areas.
The WHO reports there are about 30 species of _Hyalomma_ and that CCHF [virus] is endemic in all of Africa, the Balkans, the Middle East and in Asia south of the 50 degree parallel north.
Congo fever is, like Ebola, a viral haemorrhagic disease and the symptoms and disease progression share many similarities.
CCHF virus can be responsible for severe outbreaks in humans, but it is not pathogenic for ruminants, their amplifying host. It is the commonest viral haemorrhagic fever occurring in South Africa, with between 5 and 25 cases reported annually.
There is no vaccine available for either people or animals. Up to 30 per cent of patients who contract CCHF will die, but the prognosis is favourable in the remaining 70 per cent.

One of the 3 teaching hospitals in town [Rawalpindi, Punjab province], Holy Family Hospital [HFH], registered as many as 4 suspects of Crimean-Congo hemorrhagic fever (CCHF) within the last 10 days [approx. 10-20 Sep 2014] of which one female has been confirmed positive for the disease.
The registration of 4 suspects and confirmation of a patient has strengthened doubts that the population in the region may face a possible spike of CCHF ahead of Eidul Azha when sacrificial animals are being brought to town in bulk.
The patient tested positive for the CCHF at the HFH is a female in her 20s who is presently undergoing treatment at the hospital in isolation. The News has learnt that all the 4 patients including suspected and confirmed are residents of Tehsil Talagang in District Chakwal and have history of animal handling.
Studies reveal that the CCHF is caused by Nairovirus of the Bunyaviridae family transmitted to humans by the bite of _Hyalomma_ tick found on the skin of animals including goat and sheep or by direct contact with the blood of an infected animal or human. The case fatality rate ranges from 2-50 per cent. The CCHF was 1st described in Crimea in 1944 and identified in 1956 in Congo.
The disease was 1st reported in Pakistan in 1976, but the number of cases shows a dramatic rise since 2000, with 50-60 patients being reported annually. Nearly 50 cases of the disease have so far been reported from different parts of the country this year [2014], however, none of the concerned authorities has taken any precautionary measures to avoid a possible spike of CCHF.
The female patient, presently admitted at the HFH, is completely stable and out of danger, said Focal Person for Infectious Diseases at Rawalpindi Medical College and allied hospitals, Dr. Javed Hayat, while talking to The News on Friday [19 Sep 2014]. He added the patient has almost recovered and the hospital would discharge her within a day or 2.
To a query, he said the HFH registered a total of 12 suspects of CCHF this year [2014] of which 4 were tested positive. He suggested proper preventive measures should be taken while handling animals to avoid the disease.
It is better to establish water ponds carrying DEET or potassium permanganate and animals should be passed through it to clear their skin, said Dr. Hayat while responding to a query.
It is important that the CCHF is endemic in Pakistan and has claimed at least 10 lives within the last 5 months of which 3 patients were from the Punjab province while 3 are from Islamabad.

The [Crimean] Congo [hemorrhagic fever; CCHF] virus claimed another life in Karachi as a 51-year-old man died on Friday [26 Sep 2014] due to the deadly, tick-borne viral disease. This was the 3rd case reported in the city this year [2014], with 2 of them fatal.
Karachi Health Director Dr Zafar Aijaz told The News that the man, a resident of Liaquatabad, had gone to the sacrificial cattle market on Super Highway a few days back and had developed high fever and other symptoms related to the CCHF virus.
Aijaz said this was the 3rd case of CCHF reported in the city in the current season. According to him, one of the reported patients has recovered, while 2 of them have passed away.
The CCHF virus infects humans when they come into close contact with animals infected with ticks.

[In Karachi], 2 more patients tested positive for Crimean-Congo hemorrhagic fever (CCHF). They were admitted in hospitals for treatment and were said to be in critical condition.
A child aged 9 had been admitted in Aga Khan University Hospital with high fever. He had been diagnosed with CCHF and was being provided the required treatment. Karachi Health Director Dr Zafar Aijaz said another patient identified, aged 20, resident of Tauheedabad area in Landhi had also been diagnosed with CCHF and was being treated.
CCHF is a tick-borne viral disease, which can afflict domestic and wild animals and may also affect humans. The disease got its name after it was first described in Crimea in 1944 and later found in Congo, caused by the same pathogen.
Health experts feared an outbreak of the deadly infection in Pakistan when sacrificial animals were being brought to town from far-flung areas of the country without any precautions ahead of Eid-ul-Azha [or Eid al-Adha, Muslim Festival of Sacrifice].
A 24-year-old man who worked in a butcher's shop in Azizabad [neighbourhood of Gulberg Town in Karachi] had died of CCHF on [22 Aug 2014], the 1st case of CCHF in Sindh this year. The man was admitted to a private hospital with high fever, shortness of breath and internal bleeding 2 days before he was pronounced dead by doctors.
Experts say that in some rare cases across the world the disease has been reported to have spread from human to human.
The city reported a single CCHF case last year [2013] and 5 cases, including 4 shepherds who had shifted to Karachi from Balochistan, in 2012.
The disease was first reported in Pakistan in 1976. The number of cases showed a dramatic rise after 2000 with 50 to 60 patients being reported annually. [In Karachi], 2 more patients tested positive for Crimean-Congo hemorrhagic fever (CCHF). They were admitted in hospitals for treatment and were said to be in critical condition.
A child aged 9 had been admitted in Aga Khan University Hospital with high fever. He had been diagnosed with CCHF and was being provided the required treatment. Karachi Health Director Dr Zafar Aijaz said another patient identified, aged 20, resident of Tauheedabad area in Landhi had also been diagnosed with CCHF and was being treated.
CCHF is a tick-borne viral disease, which can afflict domestic and wild animals and may also affect humans. The disease got its name after it was first described in Crimea in 1944 and later found in Congo, caused by the same pathogen.
Health experts feared an outbreak of the deadly infection in Pakistan when sacrificial animals were being brought to town from far-flung areas of the country without any precautions ahead of Eid-ul-Azha [or Eid al-Adha, Muslim Festival of Sacrifice].
A 24-year-old man who worked in a butcher's shop in Azizabad [neighbourhood of Gulberg Town in Karachi] had died of CCHF on [22 Aug 2014], the 1st case of CCHF in Sindh this year. The man was admitted to a private hospital with high fever, shortness of breath and internal bleeding 2 days before he was pronounced dead by doctors.
Experts say that in some rare cases across the world the disease has been reported to have spread from human to human.
The city reported a single CCHF case last year [2013] and 5 cases, including 4 shepherds who had shifted to Karachi from Balochistan, in 2012.
The disease was first reported in Pakistan in 1976. The number of cases showed a dramatic rise after 2000 with 50 to 60 patients being reported annually. Nearly 50 cases of the disease have so far been reported from different parts of the country this year [2014]. However, authorities have taken no precautionary measures to avoid a possible spike of CCHF.

Following infection by a tick bite, the incubation period is usually 1-3 days, with a maximum of 9 days.

On Wednesday [15 Oct 2014], doctors confirmed to The Express Tribune that 2 people, including a staffer of Roznama Express Newspaper, have died due to [Crimean-] Congo virus in Quetta [Balochistan province].
The death toll from the virus in the city has now risen to 4 over the last 3 months.
The man was admitted to a hospital in Quetta after complaining of ill health. "He was treated for 3 days at Mission Hospital Quetta where doctors were unable to diagnose him with Congo virus," a family member of the man said. Due to deteriorating health, the patient was later shifted to Combined Military Hospital (CMH) where he was diagnosed with the virus. "It was too late to do something to save the patient after he was diagnosed with the fatal virus," family members said.
The man who was in his early 30s, was working in the production department of Century Express Newspaper Quetta.
Another young man died at Fatima Jinnah TB Sanatorium Chest Hospital. "He was brought to Provincial Sandeman Hospital Quetta on Monday [13 Oct 2014] with complaints of bleeding. He was later shifted to an isolated ward of Fatima Jinnah Hospital, where he died on Wednesday [15 Oct 2014]," family members said. Chief Medical Officer Fatima Jinnah TB sanatorium Dr Faiz Hashmi confirmed the death.
In Zhob district [Balochistan province], 4 cases of [Crimean-] Congo virus [infections] were confirmed in August 2014, 2 of whom died.

Oman's Ministry of Health has reported 6 cases of the Crimean-Congo haemorrhagic fever, CCHF, in a number of Omani governorates, and said one person died of the disease.
All of the CCHF patients had caught the tick-borne virus through contact with slaughtered or livestock animals already infected with the virus or hosting the tick vectors on their bodies, an official source from the Ministry of Health was quoted by Oman News Agency, ONA, as saying.
ONA noted that similar cases were reported over the past years in the country.
The CCHF virus, which has a fatality rate of up to 40 per cent, usually spreads to humans from cattle through tick bites or contact with meat after slaughter and can cause severe viral haemorrhagic fever outbreaks, according to the World Health Organisation, WHO.

[Cases of CCHF virus infection have occurred previously in Oman. The last Omani case reported in ProMED-mail (archive no. 20120123.1019272), in 2012, was suspected due to contact with animals illegally imported into Oman. In that post, Mod.AS noted, "CCHF virus (CCHFV) circulates in an enzootic tick-vertebrate-tick cycle; members of the tick genus _Hyalomma_ seem to be the principal vectors. The virus can be found in many species of wild and domestic mammals, including cattle, sheep, goats, pigs, horses, donkeys, and dogs. There is no evidence that the virus causes disease in animals. Domesticated ruminants including cattle, sheep, and goats are viremic for one week after experimental infections; the only symptom was a transient, mild elevation in body temperature. The introduction of viremic, symptomless animals to slaughterhouses can expose their personnel to infection. Otherwise, infected ticks are the main link between infected animals and humans; acaricides can be used on livestock and other domesticated animals to control ticks, particularly before slaughter or export.
In meat, CCHFV is usually inactivated by post-slaughter acidification. It is also killed by cooking. Unpasteurized milk should not be drunk."

Könnte bei uns demnächst Krim-Kongo ein Thema werden? Zum ersten Mal haben in Deutschland diese tropischen Zecken überwintern können.

"Nach den ersten Nachweisen dieses Jahres müssen wir davon ausgehen, dass diese Tiere bei uns in Deutschland überwintern konnten", sagte Ute Mackenstedt, die als Parasitologin an der Universität Hohenheim forscht. Im vergangenen Jahr waren erstmals Zecken dieser Gattung in größerer Menge in Deutschland nachgewiesen worden.
