Equateur province: 114 cases with 5 deaths due to monkeypox reported in Bikoro

The disease [known as] monkeypox, also known as monkey variola, has raged
in the north and south of Equateur province. At least 3 health zones are
affected. For example, in the Bikoro health zone, 114 people have
contracted the disease and 5 have died.

According to the WHO Equateur provincial branch, the Bikoro health zone has
broken the record with 114 cases. Most of the cases have been reported in
the Mooto health area, about 82 kilometers [51 mi] from Mbandaka.

Two weeks ago, medical authorities suspected the presence of a similar
disease in the Gbadolite health zone. According to the WHO/Equateur
regional medical officer, Louis Pia, laboratory tests on specimens from
patients that were sent to the INRB (National Institute for Biomedical
Research) in Kinshasa, confirmed that the cause was monkeypox virus.

The disease has been reported also in Boende health zone. The same
confirmation has been made on specimens taken from patients [in Boende]. It
is still unclear how many have been infected [overall in Equateur].

To deal with this epidemic, medical advice is for observation of hygiene
rules. The medical officer also asked farmers in the affected health zones
not pick up dead animals in the forest, including monkeys, squirrels, and
pangolins that transmit this disease to humans.

[A recent investigation suggests that monkeypox virus, to which the
smallpox vaccine also provides immunity, is now at least 20 times as common
as it was shortly after the eradication of smallpox and the relaxation of

Whereas monkeypox virus infection is somewhat less serious than smallpox,
it can still scar and even kill its victims. And in contrast to smallpox,
monkeypox is not only able to jump between humans, but can infect through
contact with the small animals that harbor the virus (such as rodents,
squirrels, and monkeys). As a result, its control may be more difficult to
achieve.

The virus species _Monkeypox virus_ is a member of the genus
_Orthopoxvirus_. Monkeypox virus can cause a disease in humans that is
clinically similar to smallpox and outbreaks in central Africa have been
associated with significant mortality (such as reported above), although
none occurred in an outbreak in the USA in 2003 associated with introduced
African rodents. An important difference between monkeypox virus and
smallpox virus (variola virus) is that monkeypox virus so far has not
transmitted from person-to-person as efficiently as variola virus. Indeed
monkeypox virus is normally resident in rodents and squirrels but can be
transmitted to other species as zoonoses. Phylogenetically the species
_Monkeypox virus_ is among the most divergent viral species from the
species _ Variola virus_. In fact variola virus is more closely related to
camelpox virus than to monkeypox virus. Consequently evolution of monkeypox
virus into a variola-like virus by mutation and selection is improbable.
Nonetheless it has potential to cause significant disease in the human
population and might require in limited situations reuse of vaccinia virus
vaccine.

Aus der Bikoro Region wird ein Ausbruch von Affenpocken berichtet, bei dem bisher 114 Menschen erkrankt und 5 verstorben sind. Auch in zwei weiteren Gesundheitszonen sind Fälle aufgetreten. Die Krankheit ist nicht so gefährlich wie die „richtigen“ Pocken, kann aber zwischen Menschen und Tieren hin-und herspringen, was eine Kontrolle erschwert. / Quelle: crm

Based on the Global Viral Forecasting and UCLA's field presence in
Democratic Republic of Congo through the USAID Emerging Pandemic
Threats (EPT) PREDICT initiative, this report of 114 cases of
monkeypox, resulting in 5 deaths, seems exaggerated. There is limited
diagnostic and confirmatory capacity for monkeypox in Equateur
Province and elsewhere in the country, outside of Kinshasa: of the
2000 suspected monkeypox cases reported across DRC in 2010, only 120
samples were taken and sent to INRB (National Institute for
Biomedical Research) in Kinshasa, of which only 20 were confirmed
positive for monkeypox.

Furthermore, monkeypox symptoms are often confused with chickenpox.
Our field team, working in collaboration with the Ministry of Health
and INRB, reported the following clarifications from Bikoro health
zone in Equateur: of the 3 samples received at INRB, 2 were confirmed
positive for chickenpox and one confirmed monkeypox. From Gbadolite
in Equateur province, of 3 samples received, 2 were confirmed
monkeypox and one was chickenpox.

Due to the capacity limitations in DRC, the Ministry of Health and
EPT partners (PREDICT, RESPOND, and CDC) and other NGOs are
recommending a national training mission through health structures,
focused on training field personnel, sampling techniques and
supplies, cold chain, and personal protective devices. One important
conclusion in collaborative meetings concerned the fact that
monkeypox is an endemic disease in DRC which often gets exaggerated
in the press. The latest news is that CDC has sent a team to Kinshasa
to prepare for a field visit to Boende, Equateur Province, to begin training.

Maculopapular lesions in the Central African Republic

In June 2010, 2 boys aged 14 years and 15 years were found to have
diffuse maculopapular lesions associated with pustules and crusts.
They had been living in a settlement in the deep forest area of the
southern Central African Republic (CAR). The cases were reported to
the Institut Pasteur at Bangui. The lesions developed after the boys
had hunted and eaten a wild rodent (bemba). They both presented to a
health centre one week after eruption of the extensive lesions, which
covered the face, torso, and limbs. Both boys were systemically well,
with no fever or notable lymphadenopathy.

Laboratory investigations were done on serum and scabs and on a biopsy
sample from the brain of a young mouse that had been inoculated 5 days
earlier with scab material from the lesions. Monkeypox virus was
detected by quantitative PCR based on partial haemagglutinin gene and
identified by sequencing. The phylogenetic tree showed that the strain
was identical to the Democratic Republic of the Congo (DRC, formerly
Zaire) strain circulating in central Africa. This reported strain was
identical to one associated with an outbreak of monkeypox in 2001 in a
Bantu family living on the border between the CAR and the DRC, 480 km
[about 300 miles] from the cases reported here.

Our patients were admitted to the local health centre and kept in
isolation until they were no longer infectious. When last seen a week
after discharge, in June 2010, both were well and neither showed any
sequelae.

Monkeypox virus has a wide range of hosts, so it can maintain a
reservoir in wild animals while sporadically causing human disease,
generally in remote villages in the rainforest areas of central and
west Africa. Large outbreaks, with transmission between people, occur
only in the DRC. The case-fatality rate in Africa is between 1 percent
and 10 percent. There is no specific treatment or vaccine although
smallpox vaccination is 85 percent effective in preventing monkeypox.
In 2003, the 1st human monkeypox outbreak in the western hemisphere
was reported in the USA after importation of rodents from Ghana.

We have describe the occurrence of 2 typical cases of monkeypox,
caused by the same viral genotype, 10 years after the previous
infections with this strain. Although monkeypox is rare, its
differentiation from other similar presenting illnesses is important.

Although monkeypox virus was 1st isolated from captive monkeys in
Copenhagen in 1958, its natural hosts are mainly rodents and squirrels
in central and west Africa. However monkeypox can infect humans and
cause lesions clinically similar to those caused by smallpox virus.
Unlike smallpox virus it does not transmit efficiently from human to
human. Sequencing of the monkeypox genome has established that in fact
it is not closely related to variola virus (smallpox virus).

Sudan in the east, the Democratic Republic of the Congo and the

A suspected case of monkeypox, was recorded in the city of Butembo, North Kivu province (eastern DRC), and was reported to the press on 15 Sep [2012] by Dr. Mundama Witende, Chief Medical Officer of the District of Butembo.

"The patient's [illness] is under control and is being monitored," he said, adding that samples were taken and sent to specialized centers. According to Dr. Mundama, people vaccinated against smallpox cannot contract the disease.

Monkeypox is a viral disease identified in primates and rodents. It is often contracted through the consumption of monkey meat and some rodents. It is transmitted to humans through the bite or contact with blood of an infected animal.

From 1970 to 1996, cases of human monkeypox were reported in DRC, Nigeria, Cote d'Ivoire, Liberia, Sierra Leone, Central African Republic and Gabon.

[Rodents, such as rope squirrels, door mice and pouched rats, are the suspected reservoir hosts, with monkeys and humans as secondary, spill-over hosts. The monkeypox virus in the Congo has a higher case fatality rate than the virus in West Africa, where fatal cases are rare or absent. Monkeypox can infect humans and cause lesions clinically similar to those caused by smallpox virus. However, unlike smallpox virus, monkeypox virus is not efficiently transmitted from person to person. Chickenpox lesions can be confused with monkeypox disease by health care workers not familiar with the differences. It will be interesting to know the results of the laboratory tests when they become available.

Butembo is a city in North Kivu in northeastern Democratic Republic of Congo lying west of Virunga National Park.

Sieben Menschen sind in Großbritannien an Affenpocken erkrankt. Wie sie sich ansteckten, ist unklar. Die Häufung ist ungewöhnlich, aber Teil eines Besorgnis erregenden Trends. Die Krankheit scheint von einem regionalen Phänomen zu einer global relevanten Infektionskrankheit zu werden ...
Am 7. Mai 2022 war bekannt geworden, dass eine aus Nigeria nach Großbritannien eingereiste Person an Affenpocken erkrankt war, eine Woche später meldeten die Behörden zwei weitere Fälle in London, die nicht mit dem ersten Fall zusammenhingen. Die vier nun identifizierten Erkrankten hatten ebenfalls keinen bekannten Kontakt zu den drei vorherigen Fällen. Das spricht für unbekannte Infektionsketten in der Bevölkerung. Nach Angaben der Weltgesundheitsorganisation haben sich alle Betroffenen in Großbritannien mit der westafrikanischen Variante des Virus angesteckt, die mild verläuft und bei den meisten Betroffenen ohne Behandlung wieder verschwindet. Die Infektion beginnt mit Fieber, Kopf- und Gliederschmerzen sowie Erschöpfung, nach einem bis vier Tagen entwickelt sich ein Ausschlag und die für Pocken typischen Bläschen und Pusteln, die verkrusten [...] Der mutmaßlich wichtigste Faktor ist allerdings, dass der bevölkerungsweite Impfschutz gegen Pocken weltweit nachlässt. Die Pockenimpfung reduziert die Wahrscheinlichkeit, sich mit Affenpocken anzustecken, um rund 85 Prozent. Allerdings steigt seit dem Ende der Impfkampagne der Anteil der Ungeimpften kontinuierlich an, so dass die Affenpocken es leichter haben, auch Menschen zu infizieren.

Erster Affenpocken-Fall in Deutschland

Nach Großbritannien sind nun auch unter anderem Deutschland, Frankreich und Australien betroffen. Hinter dem Ausbruch könnte eine neue Virusvariante stecken. Aber auch andere Erklärungen sind möglich, denn das Virus verändert sich nur langsam ...
Nun werden zwar Schlag auf Schlag neue Fälle gemeldet: Jedoch liegt dies vermutlich nicht daran, dass sich das Virus so schnell ausbreitet – sondern an der gestiegenen Aufmerksamkeit. Nachdem vergangene Woche zunächst eine Häufung von Fällen in Großbritannien beobachtet worden war, haben inzwischen unter anderem auch Portugal, Spanien, Italien, Schweden, Belgien, die USA und Kanada Fälle von Affenpocken aufgespürt. Am 20. Mai meldete Frankreich ebenfalls einen ersten Fall, zudem wurde das Virus in Australien und damit einer weiteren Weltregion entdeckt.

Insgesamt registriert wurden bis zum 30. Mai 2022, 12 Uhr, 380 Fälle in 22 Ländern. Auch wenn Experten davon ausgehen, dass es durch die Ausbreitung der Affenpocken zu keiner neuen Pandemie kommen wird, so ist die Verunsicherung doch deutlich zu spüren. Erste Länder haben mit verstärkten Grenzkontrollen reagiert, darunter Thailand, Indien, Laos, Kambodscha, Bangladesch sowie Kenia, wo bereits verstärkte Gesundheitskontrollen an sämtlichen Grenzübergängen angekündigt wurden. Rund zwei Monate nach dem ersten nachgewiesenen Fall von Affenpocken in Deutschland ist die Zahl der entdeckten Erkrankungen auf mehr als 2000 gestiegen. Das Robert Koch-Institut wies am Dienstag auf seiner Webseite genau 2033 Betroffene in Deutschland aus. Besonders viele Nachweise kommen aus Berlin, wo laut dem Landesamt für Gesundheit und Soziales bislang 1140 Nachweise des Erregers gemeldet worden sind. Mit Ausnahme von vier Frauen bundesweit sind nach RKI-Angaben alle Erkrankten Männer ...

Seit Mai 2022 greifen die Affenpocken erstmals auch außerhalb Afrikas um sich. Die Zahl der Fälle ist zwar um 90 Prozent gesunken, Entwarnung gibt es von der WHO aber nicht. Im Gegenteil ...

Bei einem großen Ausbruch der bisher auf Zentralafrika beschränkten Variante des Mpox-Virus »Klade I« sind fast fünf Prozent der Infizierten gestorben. Und das Virus ist wohl schon in Europa ...
Etwa seit 2016 allerdings verbreitet sich die in Westafrika beheimatete »Klade II« dauerhaft unter Menschen und begann 2022 eine weltweite Epidemie zu verursachen, die bis heute andauert. Treiber des globalen Ausbruchs waren Ansteckungen zwischen Männern, die Sex mit Männern haben. Mpox sind bisher ausschließlich bei engen Kontakten ansteckend, so dass bloß die relativ dichten Kontaktnetzwerke in dieser Community lange ununterbrochene Ansteckungsketten zulassen. Solche Ansteckungsketten waren bisher bei der im Kongobecken beheimateten »Klade I« nicht bekannt. Die ungewöhnlich starke Ausbreitung des Virus im Jahr 2023 deutet darauf hin, dass sich das gerade ändert. Die Zahl der gemeldeten Fälle hat sich gegenüber früheren Jahren mehr als verdoppelt. Auch in Regionen, in denen das Virus historisch nicht auftrat, werden Ansteckungen gemeldet – insbesondere seit August 2023 in der Hauptstadt Kinshasa mit 17 Millionen Einwohnern.

Die Affenpocken sind zurück – in Form einer deutlich ansteckenderen, und vor allem tödlicheren Variante. Zwei Jahre nach dem überraschenden weltweiten Ausbruch, auf dessen Höhepunkt im Jahr 2022 täglich tausende Menschen überall auf der Welt erkrankten, wächst die Gefahr einer zweiten Welle. Die Zahl der Infizierten steige derzeit exponentiell an, schreibt das Gesundheitsministerium der Demokratischen Republik Kongo, wo sich der neue Erreger derzeit ausbreitet, in einem aktuellen Bericht [...] Klade Ib dagegen scheint etwas ansteckender zu sein als der Vorgänger von 2022, es ist eine viel größere Bandbreite von Personen betroffen. Zwar scheint die sexuelle Übertragung hier ebenfalls eine Rolle zu spielen, doch es dominieren Infektionen zwischen heterosexuellen Paaren – viele Betroffene sind laut Daten der kongolesischen Gesundheitsbehörden Sexarbeiterinnen. Zusätzlich scheinen andere Übertragungswege eine Rolle zu spielen, zum Beispiel Ansteckungen im Haushalt und sogar zwischen Schulkindern.

Affenpocken | Mpox - Erste bestätigte Fälle in Burundi

Die Gesundheitsbehörden in Burundi melden den ersten Ausbruch von Mpox in Burundi. Bis zum 25. Juli 2024 sind drei bestätigte mpox-Fälle im Westen Burundis gemeldet worden, und zwar aus den Gebieten Bujumbura Nord, Gitega und Isare. Die drei Fälle werden in Gesundheitseinrichtungen behandelt. In den betroffenen Gebieten werden derzeit die Kontaktpersonen ermittelt und Nachuntersuchungen durchgeführt. Da die Überwachung der Krankheit und die Ermittlung von Kontaktpersonen fortgesetzt werden, könnten in den kommenden Wochen weitere Fälle festgestellt werden. Stand 30.07.2024.
