Ten people have died after contracting a strain of the deadly Ebola virus in the Democratic Republic of Congo, medical charity MSF said. The charity said there were a further six confirmed or probable cases in the town of Isiro. The outbreak, involving the Bundibugyo strain, was first reported last week after one person had died.

Congo DR: bushmeat blamed for Ebola outbreak

Health officials in the Democratic Republic of Congo's north eastern Orientale Province are urging the population to desist from activities that could put them at risk of contracting the Ebola virus, including contact with infected individuals and the consumption of bushmeat. At least 10 people in the province had died from suspected Ebola by 20 Aug 2012, according to the World Health Organization (WHO), which first reported the outbreak on 17 Aug 2012. Local sources say the 1st symptoms were spotted about a month ago. 9 of the deaths -- which included 3 health workers -- occurred in the district of Isiro, and one occurred in Dungu District. WHO reports a total of 15 cases so far, 13 probable and 2 confirmed. "The death case in Dungu was a hunter. Once in the bush he saw a dead antelope and did not care to ask why it had died. He immediately took [the carcass] and went to eat it... He fell sick and passed away," said the Congo DR's Health Minister Kabange Numbi.

Bushmeat -- the meat of wild animals, including hoofed animals, primates and rodents -- is often the main source of animal protein available to people in parts of the Congo DR. It is also a livelihood source for people in the Congo basin, according to a 2011 paper by the Center for International Forestry Research. According to WHO, "Ebola virus is introduced into the human population through close contact with the blood, secretions, organs, or other bodily fluids of infected animals." Fruit bats are considered to be the natural host of the virus.

"Ebola virus is an animal disease, and when people are having contact with animals in the bush, as people in some in parts of our country rely on bushmeat for their livelihood... and don't care to avoid eating meat they've got from dead animals that they often find in bushes [they risk contracting the disease]," said Mondoge Vitale, head of disease control at WHO's Kinshasa office.

The strain of ebolavirus in the Congo has been identified as Bundibugyo strain of ebolavirus -- named for a Ugandan district that borders the Congo DR. The western district of Kibaale in neighbouring Uganda was recently hit by an outbreak of the Sudan strain of ebolavirus, which killed at least 16 people. The fatality rate for the Bundibugyo strain has ben estimated at about 40 percent.

Contingency measures have been put in place to contain the outbreak. The Health Ministry has established national- and district-level taskforces and is working with partners, including the Medecins sans Frontieres (MSF), the United States Centers for Disease Control and Prevention (CDC), the UN Children's Fund (UNICEF), and WHO. "The measures include the sensitisation of the communities, protection of health workers -- in the zone where the outbreak has been declared they have been given protection equipment... We have beefed up the screening at Kisangani Port [a commercial hub in Orientale Province] to watch the outbreak across the Congo River toward the capital, Kinshasa," said Numbi. "[We are also] reinforcing screening at Kisangani Airport for passengers coming and going out to other countries' airports."

Orientale Province borders the Central African Republic, South Sudan, and Uganda. South Sudan is already on high alert following the outbreak in neighbouring Uganda. MSF has deployed experts to set up isolation facilities and surveillance systems in affected areas, but officials say their work is being hampered by a lack of information. "As long as the surveillance system is not properly enforced, we are not sure whether we may have missed some cases who have not been in contact with health facilities," said Olympia de la Rosa, a health adviser in MSF's Emergency Unit.

The Congo DR has experienced several fatal [Ebola virus disease] outbreaks in the past. In 1976, at least 280 people died from the Zaire strain of ebolavirus, which killed another 250 people in 1995 and 187 in 2007, according to the CDC.

Uganda - Ebola hämorrhagisches Fieber (EHF)

Seit Anfang Juli gibt es einen Ausbruch des hämorrhagischen Fiebers im Distrikt Kibaale im Westen des Landes, es sind mindestens 16 Menschen verstorben, etwa 23 waren erkrankt. Bei 70% der etwa 400 Kontaktpersonen ist mittlerweile klar, dass sie sich nicht angesteckt haben. Die Infektion verläuft in 50-80% der Fälle letal. Die ugandischen Gesundheitsbehörden zusammen mit Experten der WHO sowie weiteren Hilfsorganisationen haben zahlreiche Maßnahmen eingeleitet, um eine weitere Ausbreitung zu verhindern. Diese Maßnahmen zeigen sehr gute Erfolge, momentan scheint es als sei der Ausbruch erfolgreich eingedämmt worden. Die Übertragung erfolgt durch engen Kontakt (Blut, Körperflüssigkeiten) von Mensch zu Mensch. Die Infektion ist hochansteckend, eine spezifische Therapie existiert nicht. Zuletzt waren bei Epidemien in den Jahren 2000 und 2007 über 200 Menschen verstorben. Im vergangenen Jahr starb ein 12-jähriges Mädchen. / Quelle: crm

Ebola outbreak in Democratic Republic of Congo - update

The Ministry of Health (MoH) of the Democratic Republic of Congo (DRC) continues to work with partners to control the outbreak of Ebola haemorrhagic fever. As of 28 August 2012, a total of 24 (6 probable, 6 confirmed and 12 suspected) cases and 11 deaths had been reported from Province Orientale. The reported cases and deaths occurred in 3 health zones as follows: 17 cases and 9 deaths in Isiro; 6 cases and 2 deaths in Viadana; 1 case and 0 death in Dungu. The fatal cases in Isiro include three health-care workers. All alerts have been investigated and so far there has been no Ebola suspected or confirmed case reported from outside Province Orientale.

Initial samples were tested by Uganda Virus Research Institute (UVRI) in Entebbe, Uganda, and three samples taken from two patients were confirmed for Ebola virus (Ebola subtype Bundibugyo).

US Centers for Disease Control and Prevention (CDC) has established a field laboratory in Isiro to support case management and epidemiological investigations. A National Task Force convened by the Congolese Ministry of Health is working with several partners including WHO, Médecins Sans Frontières (MSF), International Federation of Red Cross and Red Crescent Societies (IFRC) and CDC to control the outbreak.

A joint MoH, WHO and MSF emergency response team is in the field to conduct a detailed epidemiological investigation and case management. WHO has deployed epidemiologists and logisticians from AFRO regional office (including the Inter-country Support team) and WHO headquarters to support the MoH and is working closely with partners in the areas of coordination, surveillance, epidemiology, logistics for outbreak, public information and social mobilization.

WHO’s Global Outbreak Alert and Response Network (GOARN) has deployed an anthropologist from the Museum National d’Histoire Naturelle (MNHN), France to assist with a clearer understanding of the social and anthropological issues among the affected population that could impact the on-going response efforts.

Control activities that are being carried out include active case finding and contact tracing, enhanced surveillance, case management, public information and social mobilization and reinforcing infection control practices.

WHO does not recommend that any travel or trade restrictions be applied to Democratic Republic of Congo.

There is currently no indication that this Ebola outbreak is related to the recent Ebola outbreak in Uganda occurring in Kibaale district.

General information on Ebola subtypes

There are five identified subtypes of Ebola virus. The subtypes have been named after the location they have been first detected in Ebola outbreaks. Three subtypes of the five have been associated with large Ebola haemorrhagic fever (EHF) outbreaks in Africa. Ebola-Zaire, Ebola-Sudan and Ebola-Bundibugyo. EHF is a febrile haemorrhagic illness which causes death in 25-90% of all cases. The Ebola Reston species, found in the Philippines, can infect humans, but no illness or death in humans has been reported to date.

The death toll from an Ebola virus disease outbreak in northeastern [Orientale province] Democratic Republic of Congo has risen to 14, and the crisis is not yet under control, medical officials have stated. The epicentre of the outbreak, which killed its 1st victim last month [August 2012], is in the busy town of Isiro in Orientale Province, but it has spread to the settlement of Viadana, 75 km away, the World Health Organisation (WHO) said in a statement. WHO also called for aid of nearly USD 2 million to help Congo's weakened health sector cope with the disease.

Last month [August 2012], officials confirmed that the epidemic involved a different strain from the one that killed at least 36 people in neighbouring Uganda and had likely been sparked by locals eating infected meat in the forest. As of Sunday [2 Sep 2012], there had been 29 confirmed or probable cases with 14 deaths, a mortality rate of around 48 per cent, Anja de Weggheleire, medical coordinator in the area for the charity Medecins Sans Frontieres (MSF) said. "A week ago, we were a bit optimistic ... but a few days later, we got some more cases ... We have quite a clear picture of what is happening, but I cannot say it is under control," she said.

Officials say that nearly 170 people remain under surveillance to see whether they develop symptoms of the disease, and with the last confirmed case on Saturday [1 Sep 2012], the end of the epidemic remains a long way off, de Weggheleire said.

Ebola is [believed to be] transmitted to humans from monkeys and birds [bats?] and [may] cause massive bleeding in victims. Mortality rates can be as high as 90 per cent.

[Since 28 Aug 2012, the number of cases has risen from 24 (6 probable, 6 confirmed, and 12 suspected) cases and 11 deaths to the current figure of 29 confirmed and probable cases and 14 deaths. 170 people are now under surveillance, although there have been no new cases since Sat 1 Sep 2012. It may be that the outbreak is being brought under control, but the origin of the outbreak remains unknown, and cases have occurred at 2 sites.

It has not been confirmed that the Ugandan outbreak originated from consumption of bush meat. Nor has it been confirmed that Ebola virus is transmitted to humans from monkeys or birds [bats?].

Ebola outbreak in Democratic Republic of Congo – update

The Ministry of Health (MoH) of the Democratic Republic of Congo (DRC) continues to work with partners to control the outbreak of Ebola haemorrhagic fever. As of 3 September 2012, a total of 28 (eight confirmed, six probable, and 14 suspected) cases including 14 deaths had been reported from Haut-Uélé district in Province Orientale. The reported cases and deaths have occurred in two health zones – 18 cases, including 11 deaths in Isiro and 10 cases, including three deaths in Viadana. The fatal cases in Isiro include three health-care workers.

All alerts have been investigated and so far Ebola has not been reported from outside Isiro and Viadana health zones. Initial samples were tested by Uganda Virus Research Institute (UVRI) in Entebbe, Uganda, and were confirmed for Ebola virus (Ebola subtype Bundibugyo). Subsequent samples have been confirmed by the field laboratory in Isiro that has been established by the US Centers for Disease Control and Prevention (CDC). Deadly Ebola virus spreads to 2nd center in eastern Congo

Congolese officials in eastern Congo say the Ebola virus disease epidemic has spread to a 2nd center after a woman was contaminated during the funeral of a victim of the disease and travelled back to her home. The health officials say 15 people have died since the beginning of the epidemic.

Congolese authorities are warning the population of the risks linked to the washing and displaying of dead bodies, a tradition that is widespread in Africa. The practice facilitates the spread of the deadly hemorrhagic fever. Around 170 people are currently under surveillance after they came into contact with infected patients.

Ebola virus disease was first discovered in Congo in 1976 and has no cure. The disease [may] causes severe internal bleeding and is fatal in 40 percent to 90 percent of cases.

[In the preceding WHO update it was stated that as of 3 Sep 2012, a total of 28 (8 confirmed, 6 probable, and 14 suspected) cases including 14 deaths had been reported from Haut-Uele district in Province Orientale. The reported cases and deaths have occurred in 2 health zones -- 18 cases, including 11 deaths in Isiro and 10 cases, including 3 deaths in Viadana. This new report increases the number of fatalities from 14 to 15 and states that the outbreak has spread to a 2nd centre. The 2nd site is not identified in this press report and only a single case is described. Further information is awaited.

Ebola outbreak in Democratic Republic of Congo – update
14.09.2012 - WHO

Retrospective epidemiological investigation recently identified additional confirmed and probable cases with Ebola haemorrhagic fever (EHF) in the Democratic Republic of Congo (DRC), according to the Ministry of Health (MoH).

As of 12 September 2012, 41 cases (9 laboratory confirmed, and 32 probable) have been reported from Haut-Uélé district in Province Orientale. Of these cases, 18 have been fatal. (5 confirmed and 13 probable). 18 healthcare workers are included among the probable cases. 28 suspected cases have also been reported and are being investigated.

Since the last update, an additional 27 cases (1 laboratory confirmed and 26 probable) of EHF were identified through retrospective epidemiological investigation in the two health zones of Isiro and Viadana. The retrospective epidemiological investigation was done to identify cases of illness which might have been EHF between the end of April 2012 and August 2012, and thus help clarify the possible origins of the outbreak and provide additional information on chains of transmission. In addition, active field epidemiological investigation is continuing to help identify all possible chains of continuing transmission of the illness, to ensure that appropriate measures are immediately taken to interrupt the transmission, and stop the outbreak.

Initial samples were tested by the Uganda Virus Research Institute (UVRI) in Entebbe, Uganda, and were confirmed for Ebola virus (Ebola subtype Bundibugyo). Subsequent samples have been tested by the field laboratory in Isiro that has been established by the US Centers for Disease Control and Prevention (CDC).

The MoH continues to work with partners to control the outbreak of EHF. A National Task Force convened by the Democratic Republic of Congo Ministry of Health is working with several partners including WHO, United Nations Children’s Fund (UNICEF), Médecins Sans Frontières (MSF), the International Federation of Red Cross and Red Crescent Societies (IFRC), US Agency for International Development (USAID) and CDC to control the outbreak.

WHO deployed epidemiologists and logisticians from the Regional Office for Africa (AFRO) including the Inter-country Support Team, and WHO headquarters to support the MoH and is working closely with partners in the areas of coordination, surveillance, epidemiology, public information and social mobilization and logistics for outbreak response. Support from the Global Outbreak Alert and Response Network (GOARN), includes deployment of an infection prevention and control (IPC) specialist and an anthropologist to assist with a clearer understanding of the social and anthropological issues among the affected population that could impact the on-going response efforts.

With respect to this event, WHO does not recommend that any travel or trade restrictions be applied to the DRC.

Since the emergence of Ebola virus infection in 1976, no Ebola virus infected patient has been able to deliver her baby alive. Now, A 29-year-old infected Congolese woman has given birth and brought world attention to the Democratic Republic of Congo (DRC).

On Thu 13 Sep 2012 at Isiro hospital, staff did not know how to deliver her baby for fear of being infected. "The staff were guided by 2 eminent practitioners by telephone from Kinshasa, Professor and Doctor Mbungu Massamba, along with the Minister of Health, Dr. Felix Kabange Numbi," says RFI. The delivery went well, and the baby is safe and sound, although his prognosis remains unclear.

[The baby is still within the incubation period for an infection acquired during delivery. So far, the news is good. This report provides no information about the health status of the mother. The photograph that accompanies the report shows the medical staff in biosafety suits with respirators, eye protection, and full body covering, including the head. We hope that they and any other staff members were equally well protected with little or no danger of acquiring the infection from the patient.

The mother was admitted to the Isiro hospital. Isiro is the capital of Haut-Uele district, Orientale province, in the northeastern part of the Democratic Republic of the Congo. Orientale province is where the current Ebola outbreak has been occurring.

Ebola outbreak in Democratic Republic of Congo – update

As of 15 September 2012, 46 cases (14 laboratory confirmed, 32 probable) with Ebola haemorrhagic fever (EHF) have been reported in the Democratic Republic of Congo (DRC). Of these 19 have been fatal (6 confirmed, 13 probable).

The cases reported are from two health zones of Isiro and Viadana in Haut-Uélé district in Province Orientale. Additionally, 26 suspected cases have been reported and are being investigated.

The MoH continues to work with partners to control the outbreak. Active epidemiological investigation is being done to identify all possible chains of transmission of the illness, and ensure that appropriate measures are immediately taken to interrupt the transmission, and stop the outbreak.

In Isiro und in Viadana in der Provinz Orientale (NO) sind im August Ebola-Erkrankungen aufgetreten. Es wurden bisher 46 Erkrankungen inkl. 19 Todesfälle registriert. Die Provinz Orientale grenzt an Uganda, wo es seit Anfang Juli eine Ebola-Epidemie gibt, die aber jetzt beendet zu sein scheint. Der Virustyp bei den beiden Ausbrüchen ist nicht derselbe, es handelt sich also um zwei unabhängig voneinander aufgetretene Ereignisse. Die WHO in Zusammenarbeit mit Gesundheitsbehörden und internationalen Hilfsorganisationen ist dabei, Eindämmungsmaßnahmen einzuleiten. Die letzten großen Ebola-Epidemien im Kongo waren 1995 (250 Tote) und 2007 (187 Tote). / Quelle: crm
