Outbreak of measles reported in Zimbabwe
----------------------------------------
Health authorities in Zimbabwe reported on Monday [14 Dec 2009] an outbreak
of measles in various provinces of the country. Most cases involved members
of registered religious groups that refuse conventional medical treatments.
To contain this disease, the authorities have launched a national
immunization campaign against measles and other preventable diseases for
children under five years of age.

WHO announces Zimbabwe measles outbreak

The World Health Organization (WHO) on Friday [18 Dec 2009] announced an
outbreak of measles in parts of Zimbabwe, a reminder of the fragile health
and humanitarian situation in this southern African country, which is

"We now have an outbreak of measles in Zimbabwe which has affected more

frustrated, as this is mainly because of people who have denied their
children vaccination," the WHO head in Zimbabwe Custodia Mandlhate told
journalists in Harare.
She said the affected areas included the districts of Bubi, Murambinda,
Makoni, Chipinge, Chirumanzu, Zvishavane and Marondera. The WHO official
said the situation in Makoni in the east of the country was "worrisome",
adding that her team was going to the areas with the intention of
vaccinating children under 5 years to slow down the spread of the disease.
Most of the cases were among members of religious groups that shunned
conventional medical treatment, while the heavy rains currently falling
over the southern African country were also hampering efforts to distribute
drugs and other medicines needed to curb measles.
The disclosure of the latest disease outbreak in Zimbabwe came as the WHO
wrapped up a conference on immunisation in Harare which drew vaccination
experts and health ministers from the region. Opening the conference on

Measles claims 22 lives in Zimbabwe

A total of 22 people, mainly children below the age of 5, have died
of measles in Zimbabwe, the country's state media reported.
The World Health Organization (WHO) said 2 weeks ago it was "deeply
frustrated" by the measles outbreak, which came after it sponsored a
vaccination program in the African nation. WHO's head in Zimbabwe,
Dr. Custodia Mandlhate, told journalists in Harare the outbreak has
totaled more than 340 suspected cases this year [2009], and "this is
not acceptable." She said the outbreak came about "mainly because of
people who have denied their children vaccination."
The Herald on Tuesday [29 Dec 2009] reported the 22 victims had not
been vaccinated, quoting health officials in the troubled country. 90
cases have been confirmed nationwide, Zimbabwe's Health and Child
Welfare Minister Henry Madzorera told the newspaper. "This is an
unwarranted loss of lives given the fact that measles is a
preventable disease," he said, according to the state-owned Herald
[newspaper]. "In Zimbabwe, outbreaks of measles were rare because of
sustained high vaccination coverage among all children. It is
therefore important for parents to realize that vaccinating their
children protects the individual children" from the disease and its
complications," he said.
The outbreak has hit 5 districts in Zimbabwe, Madzorera said. It is
likely to further strain Zimbabwe's health system.

been diagnosed widely dispersed through the districts of Bubi,

It is now reported that 90 of the 340 suspected cases of measles have
been confirmed and that there have been been 22 fatalities, mostly
unvaccinated children under 5 years of age. These figures reveal that
there has been a very serious breakdown in measles vaccination
coverage in Zimbabwe, and the outbreak is country-wide. This is
perhaps not surprising in view of Zimbabwe's fraught economy, and it
is likely that further WHO intervention will be necessary.

Seit Anfang Januar wird von einen Anstieg der Masernfälle berichtet, bis zu 4.754 Fälle wurden bereits bestätigt. Die Provinz Gauteng ist dabei mit 4.000 Fällen am schwersten betroffen, aber auch in KwaZulu-Natal und in der North West-Province nehmen die Fallzahlen zu. Nach dem südafrikanischen Impfkalender wird gegen Masern im Alter von 9 Monaten und 18 Monaten geimpft. Nach dem deutschen Impfkalender sind 2 MMR-Impfungen für alle Kinder vorgesehen. According to WHO (Zimbabwe) Harare hardest hit by measles outbreak
------------------------------------------------------------------
Harare, Zimbabwe's capital, has been hardest hit by a measles outbreak that
has so far claimed more than 40 lives countrywide since late 2009 and
accounts for 32 per cent of all reported cases, according to data jointly
released by the Ministry of Health and the World Health Organization (WHO)
on Tuesday [19 Jan 2010]. Harare had by 10 Jan 2010 recorded an
accumulative 52 confirmed cases of the disease, which normally affects
children and whose complications may lead to blindness, meningitis, or
death. At least 22 of Harare's cases were reported between 4 Jan 10 Jan
2010, raising fears that the outbreak may be worsening in the capital.
A total of 158 suspected cases have been reported in the capital since the
outbreak started in November 2009. Makoni district in Manicaland and the
Midlands' Zvishavane closely follow behind Harare, with 15 confirmed cases
each -- although the former leads in the suspected cases tally where nearly
500 reports have been investigated. Makoni also leads in terms of the
nationwide deaths recorded since the onset of the outbreak, recording 24
fatalities over the past 2 months. Mutare, also in Manicaland, has the 2nd
highest deaths, recording 15 casualties [deaths] over the same period.
An estimated 1052 suspected cases have been reported countrywide although
confirmed cases number only 161. At least 41 people have died from the
disease, according to WHO and the Ministry of Health. This is a huge leap
from the 340 suspected cases reported by WHO in Dec 2009. The head of WHO
in Zimbabwe, Custodia Mandlhate, told journalists at the time that the
hardest hit areas included the districts of [Bubi in Matabeleland North
province; Murambinda, Makoni, and Chipinge in Manicaland province;
Chirumanzu and Zvishavane in Midlands province; and Marondera in
Mashonaland East province].
Most of the cases were among members of religious groups that shun
conventional medical treatment as a matter of adherence to their faiths.
Health officials in some of the measles-affected areas have been forced to
enlist the assistance of the police to force members of an apostolic sect
to immunise their children.

Ten infants have died of measles in the past 5 days, health authorities
here told Radio VOP [a Seventh-day Adventist radio broadcast and Bible
School] on Wednesday [20 Jan 2010]. Provincial medical director (PMD) Dr
Robert Mudyiradima said the 10 included mostly children from the Apostolic
Sect religion, who are against immunization. "A suspected measles outbreak
has killed 10 people in the Murwira area in Bikita, about 10 km out of
Nyika growth point. They are believed to belong to an Apostolic Sect," Dr
Mudyiradima said.
He said The Ministry of Health deployed a team of assessors on Tuesday [19
Jan 2010] to try and contain the disease. The disease is suspected to have
spread to children following a religious gathering by the apostolic sect
members' days before. The measles outbreak comes hard on the heels of
another one in Manicaland last December 2009 which claimed the lives of 22
infants [and is still unchecked; see preceding ProMED-mail post].

Appeals for calm over measles epidemic in Bayelsa

The Bayelsa State Government on Thursday [28 Jan 2010] appealed for
calm over an outbreak of measles in Owukoro, Southern Ijaw Local
Government Area of the state. The state Commissioner for Health, Dr.
Azipabu Eruani, said the epidemic had been contained and assured
residents of the area that it would not spread to other parts of the
riverside local government or [elsewhere in the] state.
Eruani said doctors and other medical personnel, as well as the
necessary medications had been mobilised to the affected communities.
He said an aid group, Medecins San Frontieres, had also mobilised
resources to complement the efforts of the Ministry of Health. "Our
doctors are handling it. Bayelsans should be calm, we appeal to
everybody to report any health situation around them. The measles
will not spread," he said.
Eruani said 4 doctors and other medical personnel had been sent to
Owukoro alone. He said the necessary drugs and other requirements had
been dispatched to the affected community. The commissioner added
that more immunisation officers would be sent to the local government
area so that the epidemic would not spread.
He could not confirm the number of the dead at press time, saying the
claims about the number of victims in Owukoro could not be verified
by the team from the Health Ministry. "I will be going to the
community this afternoon (Thu 28 Jan 2010) to see things for myself," he added.
The commissioner stressed that the 16 new health centres that were
under construction and the 32 that were being renovated would be
completed as soon as the 2010 budget was passed into law. He said
efforts had been intensified to boost primary health care in the
rural communities, particularly with the prevailing peace in the
post-amnesty period in the state.

Cape measles outbreak

Western Cape health authorities say they are on high alert after
receiving reports that nearly 250 people around the province have
contracted measles in the past 5 months. The provincial department of
health on Monday [1 Feb 2010] confirmed that the 236 laboratory
confirmed cases of the highly contagious disease reported around the
Western Cape since September [2009] warranted describing it as "an
outbreak".
The department has already started implementing measures to address
the unusually high number of cases. It has announced that a national
mass immunisation programme, which was due to start in April [2010],
has been pushed forward and will now begin in February [2010]. Measles
is characterised by a high fever and skin rash. It mainly affects
young children. In some cases, it can cause brain damage. Flu-like
symptoms precede the first appearance of a rash.
Western Cape Health Department spokeswoman Faiza Steyn said that by
Friday [29 Jan 2010], 236 laboratory confirmed cases of measles had
been reported in the province since September [2009]. No deaths have
yet been reported in the Western Cape [province] as a result of the
disease. Out of these confirmed cases, only 10 involved people over
the age of 40. Steyn was unable to quantify how many adults had been
diagnosed with measles. The new cases, however, have been confirmed in
cases involving patients as young as 6 months old.
Both Gauteng and KwaZulu-Natal [provinces] were hit by measles
outbreaks late last year [2009]. More than 1000 children in Gauteng
contracted measles in the space of 3 months. The Gauteng outbreak also
saw one Joburg [Johannesburg] prison being quarantined because 51
inmates there had been diagnosed with measles. More than 3000 cases at
27 courts had to be postponed in Joburg as a result of the quarantine.
The Eastern Cape [province] has also experienced a surge in measles
cases during the past month. A 25-year-old man from Tshezi village
outside Mthatha died on 3 Jan 2010 after contracting the virus from a
two-year-old who came to visit during Christmas holidays from Gauteng.
By mid-January [2010], there were 50 suspected measles infections
reported from villages of Mathojanini, Thwalikulu, Mpaku, Zidindi,
Ngcwanguba, Kwaiman and Tshezi.
A team from the Eastern Cape Health Department travelled around the
province to vaccinate people against measles. And north of the [South
African] border, 22 people died in Zimbabwe during the month of
December [2009] because of a measles outbreak there.
Steyn said Khayelitsha, Mitchells Plain, Gugulethu, Athlone, and the
Cape Winelands region were hotspots for the current outbreak.
"The department can officially confirm an outbreak of measles in the
province. All health facilities have been notified to be on alert,"
Steyn said.
She said the department had first noticed an increase in recorded
measles cases in September last year [2009]. The numbers had started
climbing after the festive season, when many people were travelling
back into Western Cape province after spending time in other parts of
the country. Damaries Fritz, the chairwoman of the Cape Metro Health
Forum, has meanwhile criticised the department for "downplaying" the
outbreak. She said even though the forum was at the coalface of health
care, and was supposed to be informed of such outbreaks, it had only
learnt about the surge in measles cases on [Fri 29 Jan 2010], during a
meeting with an official from the health department. Fritz said the
forum was told by a health department official that there was an
"outbreak" in some parts of the province, particularly in Mitchells
Plain and Khayelitsha.
Steyn denied a cover-up, and said the provincial department had fed
the information to the national department for dissemination. Steyn
said a multi-phased national department of health immunisation
project, designed to tackle measles and polio, had been planned to
start in April [2010]. The project would now start this month
[February 2010], she said, and this was a direct consequence of the
Western Cape measles outbreak.

The city of Cape Town on Monday [1 Feb 2010] issued a warning about a
measles outbreak. "A measles outbreak which started in Gauteng
[province] last year [2009] has spread to all provinces in the
country, including the Western Cape," said the city's health
department spokesperson Ivan Bromfield. He said more than 100 cases of
measles had been reported in Cape Town since October [2009].
Measles is a viral infection and is characterised by a cough, runny
nose, fever, and a blotchy red rash that appears several days after
the initial symptoms. A person who contracts measles is infectious the
day before the symptoms occur, and for about 4 days before and after
the rash appears.
"The rash first develops in the facial area, with swelling of the
eyes, conjunctiva, and a redness of the mouth. The rash then spreads
over the body within 3 to 7 days," said Bromfield. After the 2nd day
of the rash, there is minimal risk for infecting others.
Measles was most severe in children who are malnourished and under the
age of one. Measles was also more severe in adults than in children
over 2 years old, with potential complications including ear
infections, diarrhoea, pneumonia, croup, and convulsions. Health
officials recommend that all children routinely receive a measles
vaccination at 9 months and a booster injection at 18 months.
"Measles immunisations are available free of charge from all city
health clinics," said Bromfield. "Measles is a preventable disease,
and the city urges residents to be aware of the symptoms and to ensure
that their children's vaccinations are up to date," he said.

Bayelsa: Measles Epidemic Death Toll Now 25

Despite claims by the Bayelsa State Government that it has curtailed
the recent outbreak of measles in Oweikurogha, Southern Ijaw Local
Government Area (LGA) from spreading to neighbouring communities, the
death toll of those affected had risen to 25. The initial outbreak of
the epidemic last week [1st reported on Thu 28 Jan 2009] reportedly
resulted in the loss of 18 lives, including children and teenagers,
according to close sources in the predominantly river side community,
who added that people are already fleeing to neighbouring areas.
The State Commissioner for Health, Dr. Azipabu Godbless Eruani while
speaking to newsmen during a briefing at his office on Thursday [28
Jan 2010] confirmed the incident, but could not give the exact
casualty figure. He said doctors with Medecins Sans Frontieres have

the immune system of the villagers, just as he expressed dismay that

Frontieres staff have also visited the community, but we have a
setback. Some people do not want to bring out their children to be
immunised when our health workers went there to stop the problem from
spreading," he added. The health boss however pointed out that the
government would mobilize staff to carry out an aggressive
enlightenment campaign in the area and other parts of the state to
ensure that people of the state understand the importance of
immunization.
Describing the measles outbreak as an international problem, the
Commissioner noted that more health workers have been deployed to
neighbouring communities so that the scourge does not spread to
neighbouring areas. He called on people of the state not to panic, as
the situation is under control.
"When this government came on board, a major focus was to improve
health care, which underscores why the Chief Timipre Sylva-led
administration is building new primary health centres and renovating
existing ones so as to bring them to standard," Dr. Eruani said. The
Daily Champion learnt that the situation is made worse as the only
government health centre in the community has been overstretched by
the influx of patients there, while the belief of the people has
precluded parents from bringing out their children, hence the increase
in death; as they believe in supernatural healing.
Close sources in the Oweikurogha community said if the health centre
there is upgraded into a full-fledged hospital with doctors and
nurses, such scourges would be readily controlled. They feared that
more people could contract diseases, hence some people sought refuge
in nearby communities.

confirm the number of the dead at that time, saying the claims about
the number of victims in Owukoro could not be verified by the team
from the Health Ministry. The above press report of 2 Feb 2010
indicates that there had been 15 fatalities at the time of the
Commissioner's statement, and that number has since increased to 25 in
the ensuing 5 days.
The local health authorities are clearly aware of the problem and the
remedy and have the assistance of Medecins Sans Frontieres. They are
hampered on the one hand by lack of facilities and on the other by
reluctance of the local population to allow their children to be
vaccinated. Education of the local population will be an essential
preliminary to successful eradication of measles virus infection in
the state.

Bayelsa: Measles Epidemic Death Toll Now 25

Despite claims by the Bayelsa State Government that it has curtailed
the recent outbreak of measles in Oweikurogha, Southern Ijaw Local
Government Area (LGA) from spreading to neighbouring communities, the
death toll of those affected had risen to 25. The initial outbreak of
the epidemic last week [1st reported on Thu 28 Jan 2009] reportedly
resulted in the loss of 18 lives, including children and teenagers,
according to close sources in the predominantly river side community,
who added that people are already fleeing to neighbouring areas.
The State Commissioner for Health, Dr. Azipabu Godbless Eruani while
speaking to newsmen during a briefing at his office on Thursday [28
Jan 2010] confirmed the incident, but could not give the exact
casualty figure. He said doctors with Medecins Sans Frontieres have
moved in to help rescue the situation from escalating.

Die Provinz Gauteng ist mit 4.000 Fällen am schwersten betroffen, zunehmende Zahlen finden sich aber auch in KwaZulu-Natal und North West Province. Auch am Westkap nehmen seit September 2009 die Masern-Erkrankungen zu. Bis Anfang Februar konnten 243 Fälle und 3 Todesfälle nachgewiesen werden. Nach dem südafrikanischen Impfkalender wird gegen Masern im Alter von 9 Monaten und 18 Monaten geimpft, 2007 lag die Durchimpfungsrate bei 83%. Bei einer großangelegten Impfkampagne wurden bis Ende Oktober 2009 3 Millionen Menschen gegen Masern geimpft. Nach dem deutschen Impfkalender sind 2 MMR-Impfungen für alle Kinder vorgesehen. 447 cases of measles in [Western Cape Province]

The number of measles cases in Western Cape Province has increased
dramatically, and now stands at 447. According to the National
Institute of Communicable Disease (NICD), there were at least 82
newly confirmed laboratory cases in the province by Tuesday [9 Feb
2010], bringing the number of cases to 447.
Gauteng [province] still has the highest burden, with 4359 cases,
followed by KwaZulu-Natal [province] with 631, North West [province]
with 563, and then the Western Cape [with 447].
In the past week, the Eastern Cape saw the highest number of newly
confirmed cases. The 92 new cases there bring their total number of
cases to 314. KwaZulu-Natal had 67 new cases and Gauteng 84, the
institute said. The Northern Cape recorded 75 new cases.
In the Eastern Cape, 3 people have died of a fatal pneumonic measles
strain since January [2010], the latest victim being a pregnant woman
who died last Wednesday [10 Feb 2010].
Last week [week ending 7 Feb 2010] the Western Cape came under fire
from the national health department and civil society groups for
withholding information about the measles outbreak in the province.
The national department said provincial health authorities had failed
in their mandate to inform other health role-players and communities
in the Western Cape about the outbreak. But the provincial
authorities hit back, saying they had kept [silent] because they did
not want to cause "unnecessary panic".
Health MEC [member of the executive council] Theuns Botha said his
administration was handling the measles issue as an "outbreak", not
an "epidemic". He said a measles and polio [vaccination] drive, which
would include radio health talks, advertorials, posters, and
pamphlets, would be fast-tracked, but could not give a timeline for this.
Health department spokeswoman Faiza Steyn said there had been an
increase in suspected measles cases in Bloekombos, and Kraaifontein
[Cape Town, Western Cape province]. She did not quantify the
increase. There had been no deaths linked to the measles outbreak, she said.
